What is Biofeedback Therapy?

Biofeedback therapy is a technique that helps a person learn to control certain body functions like blood pressure, heart rate, temperature, and muscle tension. During biofeedback therapy sessions, a patient is attached to medical equipment that monitors one of these body functions. As the patient's body sends out signals related to that function, the equipment lets the patient know how well they are doing at controlling it. This feedback helps the patient recognize how their mind influences their body and teaches them how to gain more control.

How Does Biofeedback Therapy Work?

Medical sensors are attached to a patient's skin to monitor certain body functions like heart rate, blood pressure, etc. The sensors send information to the biofeedback machine, which translates this data into a visual signal or a sound that the patient can see or hear. As long as the patient's condition remains within the desired range, the signal remains steady.

When the patient's condition goes out of the desired range, the signal changes. This tells the patient that they need to change their physical and mental state to regulate that function and keep the signal stable. By paying close attention to body sensations and feedback signals, patients can learn to regulate and gain more control over that body function.

Over multiple biofeedback therapy sessions, patients slowly learn the skills to regulate and modify their body functions with the help of their minds. Eventually, they become better at maintaining these functions even without biofeedback equipment.

Principles of Biofeedback Therapy

Biofeedback works on three simple but important principles: physiological interconnection, operant conditioning, and self-regulation. Let's look at each one and how they apply to biofeedback.

Physiological Interconnection

All parts of our body are interconnected. What happens in one area physically affects other parts. For example, when we get angry, our heart rate goes up, muscles become tenser, and breathing quickens.

Biofeedback works by monitoring one physiological process like heart rate, muscle tension, or skin temperature. The aim is to help patients learn that they have the power to influence and control that process through their minds. Patients realize the connection between their mental state and physiology.

Operant Conditioning

Operant conditioning is a learning process where behaviors are shaped by positive and negative reinforcement. Biofeedback employs this principle to teach patients how to control their body functions.

During biofeedback, when a patient's physiological process moves towards the desired goal, there is positive reinforcement through a signal change. But when the process moves away from the goal, there is no negative reinforcement.

Over time, patients learn to associate certain mental techniques with getting a positive reinforcement signal. They keep repeating the thoughts and actions that produce that signal. This gradually conditions them to control that body function on their own.

Self-Regulation

The aim of biofeedback is to teach patients how to self-regulate certain body functions. Initially, biofeedback machines provide feedback to guide self-regulation. But the ultimate goal is for patients to regulate themselves without the equipment's help.

Through biofeedback training, patients realize they are not helpless victims of their body processes. They gain confidence in their abilities to manage those processes with their mind. Patients learn how relaxing certain muscles, changing breathing patterns, or altering thoughts can influence their body functions.

Learning self-regulation skills allows patients to manage stress, reduce anxiety and cope with health conditions more effectively on their own. And once learned, these skills tend to persist long after biofeedback treatment ends.

How the Principles Work Together?

All three principles work together to make biofeedback an effective therapy:

  • The principle of physiological interconnection tells us that changing one body function can affect others. This shows patients that gaining control over one process is possible and worthwhile.
  • Operant conditioning shapes patients' behaviors through positive and negative reinforcement from the biofeedback machine. This helps patients learn the mental techniques to control their physiology.
  • As patients gain more control through biofeedback training, they start to self-regulate that physiological process independently. They realize the power of their mind over their body. Self-regulation skills persist long after treatment ends.

Together, these three principles can alter patients' experiences of their conditions and symptoms for the better. They learn new ways of managing stress, relaxing muscles, and influencing bodily processes that benefit them for years.

Biofeedback Techniques For Gaining Control

Heart rate biofeedback is the most common type. Electrodes are attached to the patient's chest to monitor heart rate signals. 

  • As the patient's heart rate changes, a visual display or auditory signal informs them. The goal is for patients to learn how to lower their heart rate through techniques like deep breathing, muscle relaxation, and positive thinking. This helps treat conditions like hypertension, anxiety, and stress. 
  • Patients practice these techniques while watching or listening to their heart rate on the biofeedback monitor. Over time, they learn to adjust their mental and physical state to reach the target heart rate. Eventually, they can lower their heart rate on command even without the biofeedback equipment. Heart rate biofeedback requires several sessions over weeks to show results.
  • This technique monitors the electrical activity of muscles using electrodes called electromyography or EMG sensors. The aim is to teach patients how to relax tight, tense muscles. As muscle tension increases, the biofeedback monitor produces a signal that alerts patients. They then practice relaxation techniques to reduce muscle tension and stabilize the signal. Over time, patients learn how to recognize and quickly release muscle tension on their own. This technique helps treat tension headaches, migraines, and fibromyalgia.
  • This technique tracks changes in a patient's finger temperature using a thermometer or temperature sensor. The aim is to teach patients how to raise their temperature by concentrating and relaxing. As a patient's temperature rises closer to the target range, the biofeedback monitor signals this change. Patients learn to associate mental relaxation techniques with the signal. As they practice and master these skills, patients can increase their temperature outside of biofeedback therapy sessions too. Temperature biofeedback helps conditions like Raynaud's syndrome, anxiety, and insomnia.
  • Neurofeedback tracks brain wave patterns through electroencephalogram (EEG) sensors attached to the scalp. The aim is to help patients modify abnormal brain waves associated with conditions like ADHD, anxiety, and epilepsy. When patients produce healthier brain wave patterns, the biofeedback monitor rewards them with visual or auditory signals. Over time, patients learn to replicate these optimal brain states on command. Neurofeedback requires learning how to focus attention, relax and change mental states in order to change brain wave patterns.
  • In addition to these common methods, biofeedback also uses electromyography (EMG) sensors to monitor and help patients gain control over pelvic floor muscles for treating conditions like incontinence.
